10. 3 Recommendation letters
11. Application form (collected from the hospital)

Submit your documents directly to UPTH at the address above. The hospital’s application fee, if any, is paid through Remita to the hospital’s Treasury Single Account, never to an individual.

There is no application fee for the internship itself, and the hospital does not use agents. Apply directly to the address above or through the hospital’s own portal; nobody needs to be paid to “submit” your documents for you.
